Please FAX your completed order form to ProFoldin at (508) 845-9258. You can use the order form in the format used by your purchasing office. You can also print out an order form by clicking  Order Form.  For US customers, please provide your Tax exempt number if the purchase is eligible for sales tax exemption.  For non-US customers, import tax and fees will be pre-paid by ProFoldin and billed to customers by ProFoldin.
